If you want to get an ID for your dog, you need to do following:
First you need to go to the animal disease control and prevention centres near your place to get vaccination for the pet.
Then go to the nearest police station to find the animal register department with the vaccination certificate and the owner's original passport, the copy of passport image page, residence lease copy , the sterilization certificate(got from the vet), to get your pet registered.
After that they will give you a paper note indicates when you can come to pick up the ID for the pet. We know one animal disease centres in Song lin road No.21, near Shangcheng lu, and the phone number is 021-58409444.
Please note the animal disease centre and the police should be near the place you are living. And when you go to the animal centre, you also need to prepare the same documents as you go to the police station.
