To ensure your withdrawal request is processed smoothly, please fill out the form according to the following requirements:
提交申请日期：填写您录入生物信息的日期
Date of Submission of Application: Date of enrolling biometrics
收据登记号码：填写BLS收据上的参考编号
Registration number as per receipt: Reference number mentioned on the BLS receipt
申请人姓名：必须与护照上的姓名完全一致
Applicant Name: As per passport
出生日期：必须与护照上的出生日期完全一致
Date of Birth: As per passport
签证类型：与您原申请表中填写的签证类型一致
Visa Type: As per application form
