Documents Required：
Your ID photo, Medical-test Letter of Attorney, the Renew/Post-register/Re-register Dealing Record, a photocopy of your passport homepage. ( If you ask another person to deal with this, his or her ID card and the photocopy are also needed.)
Procedures:
I.Preparing Necessary Documents
Fill out the Renew/Post-register/Re-register Dealing Record (Having forgotten the specific time when you did the checkup? Go to the counter and show your passport homepage photocopy, then the officer will help query the exact day!)
If you commission another person to handle it, you need to print a Medical-test Letter of Attorney in Chinese (with your signature or name stamp ). The format should follow as below:
Ⅱ.Pay the Fee
Find Dr.Zhou and ask for his signature in Room 312, Floor 3rd, Quarantine and Inspection Bureau (the opposite of Medical Examination Center) before 15:00.
Pay for 60 RMB at ground floor and hand in your ID photo to the officer.Then, you will get your new health report about an hour later!
Tips:
Remember to prepare all the required documents with your signature on them.
Ask for Dr.Zhou’s signature before 3p.m.
Do not forget your ID photo (you can’t apply for a new report without it)!
60RMB for one report, 90RMB for two.
