Important: Declare Self-used Articles To the Chinese Customs!

Permanent personnel, non-resident permanent foreign passengers, overseas Chinese (including HK, Macao and Taiwan residents), travelers who settle in China and other high-level overseas talents can download the Form from the official website of General Administration of Customs, then fill it on their own.

Foreign visitors, Chinese residents who travel abroad, overseas students who come back to China need to go to Customs offices to get the Form (in quadruplicate).

Full name coincided with the name on your valid passport.

 • The same as the No. on the Residence Permit for Foreigners in the PRC.

 • People who are required to fill out the Form in quadruplicate need not to write the No.

 • The same as the No. on the Foreigner’s Work Permit Card (or Alien Employment Permit), or Foreign Expert Certificate.

• People who are required to fill out the Form in quadruplicate need not to write the No.

You need to fill in details according to specific items. For example:

 • Air conditioner → write down its electric power

 • Television, displayer → write down the screen size

 • Refrigerator, wine cabinet → write down its specific volume

 • Container → write down its capacity of storage

Only the permanent personnel need the seal of organization, and it should be coincided with the name of the Organization which has been mentioned in the Form before.
