When COVID-19 struck, it struck hard!
It struck with a force destabilizing people’s lives and it caught all of us off-guard.

When the COVID-19 epidemic ran rampant in the streets of Wuhan and subsequently overflowed to the
surrounding regions, Shanghai did its part in all sincerity and the residents followed likewise. People stayed at home… anxious, worried, perturbed… checking on different media portals, looking for some hope,
praying, hoping…
But for a 35 years old Ms. X (name withheld for patient privacy issues), none of those happenings
mattered… as she lay on her own pool of blood on a hospital bed. She had just given birth to a beautiful baby when she started bleeding. And very soon, the battle to save her life would suddenly turn into a race against time. Her pulse bounded and her blood pressure dropped as the persistent massive bleeding of a blood volume that is unimaginable put her hanging on a delicate balance between life and death. Every minute mattered.
The hospital placed a call to Shanghai Blood Center, who in turn initiated the Green Channel. Mechanisms rolled into place to leave no stones unturned to save the life of Ms. X. After more than three hours of a ”bloody struggle”, the demand for blood was met.
Ms. X opened her eyes, and with tears rolling down her eyes, she thanked the medical personnel around her and remotely thanked the blood donors. She understood very well that the selfless contribution of the donors had given her family hope, and her baby a better tomorrow with a mom present.

Thanks to the collective efforts of the medical workers, staff at Shanghai Blood Center and the gracious blood donors, Ms. X had been lucky to survive.
Ms. X's story is a true one that happened very recently.
But what is to happen to the next patient? Will she be as lucky?
The outbreak of COVID-19 was on such a scale that none of us had even remotely imagined this scenario even a couple of months ago.
With the drastic reduction of blood donors, the supply of blood products is significantly low. In the face of a sudden huge blood demand , Shanghai Blood Center (SBC) has been coordinating through multiple fronts to meet the demands.
I know staff at SBC… some of them very well. They have been taking turns to donate blood themselves in the face of this shortage. Their struggle is real, and their sacrifices are praiseworthy.


So while most of us are placed within the confines of our own spaces… for that man who had an accident and suffered internal bleeding from trauma, for that child who is receiving chemotherapy and needs platelets, and for women like Ms. X who just gave birth and unfortunately had post partum hemorrhage, they have no choice. COVID-19 is killing them… indirectly.
And for those patients that COVID-19 is killing directly, why is blood donations necessary?
2019-nCoV in severe cases causes respiratory symptoms leading to pneumonia which can further proceed to complications like respiratory failure, Multiple Organ Dysfunction Syndrome, MODS (alteration in organs function in an acutely ill patient with loss of the body’s delicate internal balance) and Disseminated Intravascular Coagulation, DIC (abnormal clumps of thickened blood form inside blood vessels which use up the blood's clotting factors leading to serious bleeding in other places).
Thus, while there is a regular requirement in any affected community of blood for hemorrhage from trauma, bleeding disorders, complications of pregnancy, major surgical procedures and other conditions requiring blood, now there is that extra burden from the extra load of the COVID-19 pneumonia patients.
In patients with novel coronavirus pneumonia who become critically ill, extracorporeal membrane oxygenation (ECMO) may be needed to treat respiratory failure, and this may require transfusions of large volumes of red blood cell components. Patients with Multiple Organ Dysfunction Syndrome (MODS) or Disseminated Intravascular Coagulation (DIC) may require fresh frozen plasma (FFP), cryoprecipitate and other blood components to save lives.

Shanghai Blood Center has implemented strict protective disinfection measures to ensure safety of blood donors.
Thermal screening on arrival.
Regular use of hand washing and hand sanitizers.
Use of protective facial masks.
Questionnaires for thorough screening.
Coordination with donors to present at different times to prevent crowding.
Regular ventilation and cleaning with disinfectants.
Minimizing contact with contact precautions.
Meticulous supervision during the donation procedures.
