研究摘要
糖尿病视网膜病变(DR),是糖尿病(DM)患者最突出的微血管并发症之一。糖尿病眼病(包括DR和糖尿病黄斑水肿(DME))的全球患病率增加与糖尿病患病率飙升密切相关。尽管深度学习技术已用于基于视网膜眼底照片特征的DR和DME的自动检测,但并不了解其分子层面的表达差异。本研究旨在研究血浆细胞因子的变化,并开发用于预测2型糖尿病患者非增生性糖尿病视网膜病变(NPDR)的机器学习分类器。
研究思路
IDEAS
1、样本收集分组和蛋白芯片实验
首先选取14例DM病人和14例NPDR病人,通过他们的血液样本进行蛋白芯片(GSH-ANG-2 & GSH-ANG-3,两款30个人血管生成因子半定量抗体芯片,共60个指标)实验,找到两组间表达的差异蛋白。


通过芯片实验结果每种蛋白的表达量绘制热图、火山图和PCA图等,加以限制条件(FC>4或者FC<0.25)进行筛选差异显著且差异倍数大的蛋白。Pearson相关分析,分析临床特征与差异蛋白的相关性。对于NPDR,PDGF-BB是与糖尿病持续时间(r = 0.34)弱相关,VEGFR2与总胆固醇(r = 0.33)和低密度脂蛋白(r = 0.30)弱相关。

2、差异蛋白的验证
蛋白芯片筛选出的六个差异蛋白PDGF-BB, TIMP-1, TIMP-2, ANG-1, CXCL16和VEGFR2用验证集的大量样本(DM:115;NPDR:115)通过ELISA进行验证。NPDR组中ANG-1, PDGF-BB, TIMP-2, 和VEGFR2要显著高于DM组。而CXCL16和TIMP-1则未通过验证,无显著差异。

3、建模特征蛋白的选择
接下来进行特征蛋白的降维,选取PCA结果的第一主成分,其中包括6种蛋白CXCL16、TIMP-1、ANG-1、TIMP-2、PDGF-BB、VEGFR2对整体方差的贡献最大,随后用两种算法随机森林和lasso回归找到更为关键的三种蛋白(PDGF-BB、VEGFR2、ANG-1)进行后续的建模。

4、建模特征蛋白的选择
用这三个蛋白进行多种模型(神经网络,逻辑回归,支持向量机,XGBOOST和随机森林)构建然后评估每个模型的准确率。可以通过每个模型的AUC值或者准确率或者F1值进行评估,最终找到表现最好的随机森林模型,预测的准确率达到85%。


结 论
Conclusion
研究利用RayBiotech抗体芯片(GSH-ANG-2 & GSH-ANG-3)检测出NPDR患者中血浆中ANG-1,PDGF-BB,TIMP-2和VEGFR2的血浆细胞因子增加,并且从血浆细胞因子中基于机器学习算法找到NPDR分类器,即从DM患者中进行对NPDR的预测。血浆细胞因子可能是NPDR的强风险标志物。
